1. Assumption of Risk
Participation in Elite Performance Futbol ("EPF") training sessions, clinics and programs involves physical activity and inherent risks, including but not limited to collision with other players, falls, strains, sprains, fractures, and other injuries. By signing below, the parent/guardian acknowledges these risks are understood and voluntarily accepted on behalf of the participant.
2. Release & Indemnity
To the maximum extent permitted by law, the parent/guardian releases and indemnifies Elite Performance Futbol, its coaches, staff, and Doohan Reserve / Oran Park venue operators from any claim, loss, damage, or liability arising from participation in EPF programs, except to the extent caused by the gross negligence or wilful misconduct of EPF or its staff.
3. Medical Fitness
The parent/guardian confirms the participant is medically fit to take part in football training and has disclosed any relevant medical conditions, injuries or allergies on the registration form. EPF relies on this disclosure being accurate and complete.
4. Medical Treatment Authorisation
In the event of injury or medical emergency during a session, the parent/guardian authorises EPF coaches and staff to arrange first aid and, if necessary, call for ambulance or emergency medical services on the participant's behalf. EPF will make reasonable efforts to contact the parent/guardian as soon as practicable.
5. Photography & Media
EPF may take photos or video of sessions for coaching, promotional or social media purposes. Parents/guardians who do not consent to this should notify EPF in writing prior to the participant's first session.
6. Code of Conduct
Participants and parents/guardians agree to follow reasonable direction from EPF coaches and to treat other players, coaches and venue staff with respect. EPF reserves the right to suspend or end a participant's enrolment for conduct that endangers others or repeatedly disrupts sessions.
7. Acknowledgement
By digitally signing on the registration form (or signing below in person), the parent/guardian confirms they have read, understood, and agree to this Liability & Waiver on behalf of the named participant, and understand that an original signed copy is required at the participant's first session.
